Procedures

1. The applicant shall submit the old card and the enclosed documents to the Reception Desk officer.
2. The letter of renewal and the enclosed documents will be verified for completeness.
   1. If the submitted documents are not complete, the applicant will be asked to update the required documents.
   2. If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
3. The auditor endorses the voucher.
4. The applicant pays the prescribed fees at the cashier’s counter.
5. Submit the form and the enclosed documents to the Reception Desk officer
6. The Applications Intake officer enters data.
7. The auditor receives the application and validates information against it.
   1. In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
   2. In case the application is complete, the Audit and Posting officer will post the application.
8. The application is sent electronically to printing department.
9. The new card is printed and handed over to the Reception Desk officer.
10. The Reception Desk officer then gives the new health card to the applicant.

Required Documents

• A cover letter from the ministry, institution or the authority in which the applicant is an employee.
• The old cards.
• One (1) personal photograph for every beneficiary and subscriber.
• A recent salary statement showing the deduction for the health insurance allowance.

Persons benefiting from the service

Ministers, members of Senate, representatives, employees of government departments, municipalities, public institutions and official bodies, Greater Amman Municipality‎, Per-diem workers, their wives or husbands and children under (18) years.
